You must of missed the various attempts with screenshots to replicate the issue, and we couldn't. We also know the thousands of people who have used this same NPC to refine the same item, and they did not encounter it. We are not going to waste resources trying to fix a working npc, if you can just as easily follow the suggested usage procedure to avoid items during refinement.
